An internal incident notification, an OSHA recording decision and a report to OSHA are separate evidence states. A data center workforce coordinator should route each required review to the responsible employer and safety owner, rather than treat one completed incident ticket as proof that every obligation is closed.
This is an administrative coordination aid. It does not classify an actual injury, provide medical or legal advice, decide an individual's employment or authorize work. Keep sensitive case details in the approved restricted system and use controlled references in coordination records.
What do recording and reporting refer to?
OSHA's general recording criteria in 29 CFR 1904.7 concern the injury and illness recordkeeping decision. 29 CFR 1904.39 separately addresses reporting specified severe work-related events to OSHA, with its own timing, definitions and exceptions. A log entry is not itself the reporting receipt.
The responsible employer must assess applicability, work-relatedness and the actual event under the complete requirements. Applicable State Plan requirements and the project's client-notification rules also need their own check. Do not postpone a time-sensitive reporting review until the next routine dashboard update. Cortex Construct is independent and is not affiliated with OSHA.
Which evidence tracks should stay separate?
The coordination copy should identify owners and statuses without duplicating medical information. An unresolved classification is not a zero incident count or proof that reporting is unnecessary. A client-required internal notification also does not replace a regulator submission.
| Track | Evidence reference | Closure question |
|---|---|---|
| Internal notification | Employer's controlled incident reference | Did the responsible owner receive it? |
| Recording review | Authorised recording decision | Was applicability assessed and the decision recorded? |
| Severe-event review | Responsible safety owner's reporting assessment | Were the actual reporting rules and timing considered? |
| Submission | Actual regulator receipt where required | Is there evidence of the required submission? |
| Client requirement | Contract or site notification reference | Was that separate requirement addressed? |
| Follow-up | Assigned action and documented review | Is the action actually closed? |
This table does not imply that every event requires every submission. The owner records the applicable decision and its controlled basis. An empty receipt field should be explained as pending, not applicable under a reviewed decision, or missing evidence, rather than silently marked complete.
How can one closed ticket leave another track open?
Illustrative example. A fictional coordination dashboard shows an internal incident ticket as closed because the initial notification reached its owner. It contains no reference to the employer's recording review or any required reporting assessment. The coordinator leaves those evidence tracks unresolved and contacts the designated safety owner through the established process.
The coordinator does not infer an actual injury classification, submit a regulator report from incomplete information or rewrite the dashboard as no report required. If the owner later supplies a controlled decision or submission receipt, the coordination record links it with its actual date and scope. The example contains no real worker or medical information.
Keep corrections and later information traceable. Closing an administrative evidence gap does not establish that the original event was handled on time, nor does a reporting receipt establish that every corrective action is complete.
How does this connect to workforce handover?
The hazard-reporting interface guide concerns the route for raising a hazard. The task-change and JHA review concerns the current task analysis and briefing. Incident recording and reporting evidence should have its own owner and references alongside those processes.
Use only approved aggregate information for general workforce planning. Do not use this worksheet to rank workers by medical history or decide who may be employed. Site safety owners control applicable incident processes, and engineering sign-off is performed only by the registered engineer on the job.
Frequently asked questions
Does an internal incident ticket prove an OSHA report was made?
No. Keep any required regulator submission and its actual receipt as separate evidence.
Does a recording decision replace the severe-event reporting review?
No. The responsible owner must consider the applicable requirements for each track.
Should personal medical details be copied into the coordination register?
No. Use approved controlled references and the project's restricted information process.
